Melasma :-

Melasma is a common pigmentary disorder of skin. In which brown & blue-grey patch appear mainly on face.

Also known as ‘mask of pregnancy’

Pathophysiology :-

Melasma pathophysiology is multifactorial & not yet entirely clear.

Relation with female hormonal activity seems to be present.

Clinical feature :-

Brown & blue-grey macular pigmentation with well defined margins (mainly involving forehead, cheeks, nose)

Types :-

epidermal :- has a dark brown colour, a well defined border. Response well to treatment

Dermal :- light brown to bluish colour, difficult to treat

Mixed :- has both bluish & brown patch. Difficult to treat.

Treatment :-

1) Sun-protection :- sunscreens are most important part of therapy.

Sunscreen with at least spf 30 are advised. Tinted sunscreen are more preferred choice.

2) Tyrosinase inhibitors :- hydroquinone, retinoids, kojic acid, arbutin, vitamin-c, glutathione, azelaic acid etc

Inhibitors of melanosomes transfer :- niacinamide, retinoid, soy

Increase skin turnover :- retinoid, glycolic acid, lactic acid, azelaic acid, mandelic acid etc

3) advance treatment :-

Chemical peeling :- removes unwanted melanin by causing a controlled chemical burn to skin
Photo-facial :- targeted at the melanin in the skin's surface and destroys melanin to remove the discolouration.
Laser facial (Q-swt laser) :- breaks the melanin pigment which then drained via lymphatic system led to pigment reduction & also stimulate collagen production.
